The decision and its context. What are you deciding, and what is the situation in which you're deciding it? Not the full background, but the specific conditions that feel relevant: time pressure, competing priorities, information quality, who else is involved.

The reasoning. Why does this option look better than the alternatives? What are you believing about the world — about the market, about the person, about the organisation — that supports this choice? Forcing yourself to state the reasoning explicitly is valuable even before any outcome review, because it surfaces assumptions that would otherwise remain implicit.

The upside and downside cases. What does a good outcome look like? What does a bad one look like? What conditions lead to each? This matters for two reasons: it disciplines your thinking at decision time, and it gives you something specific to test against when the outcome arrives.

The risks you're accepting. Not a comprehensive risk register — the specific risks you're aware of and have decided don't disqualify the decision. The logic for discounting them is often the most valuable part of the record. When decisions go wrong, it's almost always because a visible risk was underweighted. Capturing why you weighted it the way you did is how you recalibrate.

Your confidence level. On a consistent numerical scale. Not "I feel good about this" or "cautiously optimistic" — a number that you can compare across decisions and over time. Confidence scores only produce insight when they're on the same scale and captured consistently. One-to-ten works well.

For some decisions the horizon is short; for others — a strategic hire, a major investment — it's long. The checkpoint needs to be set in advance, at the appropriate horizon, with the specific questions that will be answered at that time.

Second, the outcome review needs to go back to the original reasoning, not just evaluate current performance. The questions that produce learning are: Did the upside case I described materialise? Were the risks I discounted the ones that proved relevant? What did I believe that turned out to be wrong? Was my confidence level warranted?

If you log confidence scores consistently and score outcomes on the same scale, you can eventually see the pattern: where your confidence runs above your outcomes, where it runs below, and what conditions predict each.

What volume of decisions you need
